Summary
The facility failed to ensure that call lights were within reach for a resident with significant physical and cognitive impairments. The resident, who had left-sided paralysis following a stroke and moderate cognitive impairment, was observed in bed with the touch pad call light out of reach. The resident reported that the call light was out of reach two to three times a day, causing frustration.
