Summary
Facility staff failed to notify a resident's responsible party of a significant change in condition. The resident, who was admitted with Alzheimer's disease and assessed as high risk for wandering and elopement, was observed on the day of admission making several attempts to leave the unit. In response, staff placed a Wander Guard device on the resident's right ankle.
