Summary

The facility failed to provide the required Skilled Nursing Facility Advance Beneficiary Notice (SNF ABN), Form CMS-10055, to two residents prior to the discontinuation of their Medicare Part A services. For both residents, records showed that Medicare Part A skilled services were ended while benefit days remained, and each resident was discharged home. There was no documented evidence that either resident or their responsible party received the SNF ABN before discharge.
