Unsafe Food Storage, Temperature Monitoring, and Kitchen Sanitation

Summary

The facility failed to provide a safe, sanitary environment for food production and storage. During the initial kitchen tour, surveyors observed staple food bins that were unlabeled and undated, commercial mixes removed from original packaging without an opened date, and personal items including a backpack and jacket stored in the dry storage room on top of canned and packaged foods. Surveyors also found missing temperature documentation on the tray line and on the walk-in refrigerator and freezer logs. Additional observations showed multiple food safety and sanitation issues in the kitchen. A mixing bowl, frying pans, and a clear bin of utensils were stored turned up and uncovered in the production area near the pot and pan sink, and chopping boards were stored on a lower shelf without protection from splash. On shelves near the sink, surveyors observed opened and undated cornmeal, flour, cake mix, hot sauce, beef flavoring, biscuit gravy mix, white grits, quick oats, shredded cheese, and sprouting potatoes. In the dry storage room, surveyors found opened and undated ingredient bins of thickener, pasta, and rice, dented cans of butter beans, and multiple bags of cake mix without dates. Staff personal items, including a purse, mobile phone, grocery bag, backpacks, a water bottle, a sweat jacket, and an apron, were also stored in the dry storage area among food items. Surveyors further observed environmental and temperature control concerns. The kitchen ceiling had dripping condensation and peeling or chipping gray paint near the vent between the freezer door and tray line. In the nourishment refrigerator on one unit, an opened Med Pass nutritional shake had expired and had no opened date marked. Another nourishment refrigerator temperature log showed an acceptable range of 36 degrees F to 46 degrees F, which differed from the facility policy requiring temperatures at or below 41 degrees F, and the refrigerator was recorded at 42 degrees F. Interviews with the DM, Maintenance Director, DON, Infection Prevention Nurse, and Administrator confirmed awareness of several of these conditions, including the importance of dating opened foods, avoiding use of dented cans, keeping personal items out of food storage areas, documenting temperatures, and maintaining safe refrigerator temperatures.
