Medication Storage and Labeling Deficiencies

Summary

Drugs and biologicals were not stored and labeled in accordance with accepted professional principles. During review of medication cart one with a QMA, multiple residents had medications that were missing required open dates and expiration dates, including Atrovent inhalers for one resident, Dorzolamide eye drops and multiple Latanoprost eye drops for another resident, Fluticasone nasal spray without a cap, Albuterol inhaler and oxymetazoline nasal spray for another resident, two Fluticasone inhalers with remaining puffs for another resident, and Fluticasone nasal spray for another resident. One resident also had a Lantus insulin vial found expired, and another resident had a Lorazepam vial in the refrigerator with an open date from several months earlier. The QMA stated that eye drops and nasal sprays should have open and expiration dates when opened, and that topicals should not be stored with oral solutions, eye drops, or nasal sprays. The review also found medications stored together inappropriately on the cart and in storage areas. Clotrimazole topical solution was stored with eye drops, acetaminophen oral solution was stored with topical creams, medical grade honey cream was stored with inhalers, and hydrocortisone cream was stored with oral medications. The DON stated that topicals should not have been stored with oral solutions, and that the expectation was for staff opening eye drops, nasal sprays, or inhalers to place open and expiration dates on the containers. The RNC stated that cycle refilling sometimes resulted in multiple new medications being received before the prior supply was finished, leading to multiple open medications if staff did not pay close attention.
