Insufficient Staffing and Delayed Resident Care in Memory Care Unit

Summary

The facility failed to provide sufficient nursing staff to meet the needs of residents in the secured memory care unit. During a continuous observation, only one CNA and one LPN were present on the unit while multiple residents needed assistance with ADLs, toileting, and meals. Several residents were observed with unmet needs, including a resident with a strong urine odor while asleep on a couch, residents with unbrushed hair and long nails, and residents who required total assistance or constant reminders to eat but were left without help at various times because staff were occupied with other residents or had to leave to get supplies or respond to other needs. During lunch observations, staff were unable to consistently assist all dependent residents at the same time. Residents who required total assistance or constant encouragement were left waiting while staff helped other residents, and it took over an hour to feed the totally dependent residents. One resident waited over 30 minutes to be taken to the restroom and was found incontinent with a soiled brief when finally assisted. Other observations showed residents waiting for toileting assistance while staff were already occupied, and at times there were 12 unsupervised residents while both aides and the nurse were behind closed doors helping other residents. The staffing concerns were also reflected in interviews and grievances. Family members reported residents were often not dressed as preferred, were served last at meals, and did not receive timely bathing, oral care, hair brushing, or nail trimming. Staff and leadership acknowledged that the memory care unit often had only one or two CNAs, that they could not get everything done, and that staffing was based on census. The report also described a Hoyer lift transfer in which only one person handled the lift, it tipped, and the resident was dropped into a chair and sustained a forehead cut. Resident council and grievance records also documented delayed call light response, delayed toileting, delayed bathing, and staff being too busy to provide showers.
