Summary

The facility failed to prevent an accident when a resident with Alzheimer’s disease and unspecified dementia was transferred with a Hoyer lift by only one CNA, even though the resident’s care plan indicated she required at least two staff members for mechanical lift transfers. During the transfer, the lift tipped over and the boom made contact with the resident’s forehead, causing a small cut. The resident’s husband filed a grievance stating that only one person was handling the lift when the resident was dropped into the chair and injured. The facility also failed to manage elopement-related safety concerns for a resident on the secured memory care unit who was repeatedly observed wandering in and out of his room and asking staff why he was at the facility and when he could go home. His record showed diagnoses including Alzheimer’s disease, anxiety, adjustment disorder, and repeated falls, and an elopement risk assessment identified him as at risk for elopement. His comprehensive care plan lacked implementation or interventions for elopement risk. Surveyors could not locate an Elopement Binder at either nurses’ station or elsewhere on the unit, and a bright red binder labeled with door codes was observed accessible over the nurses’ station counter with the correct codes taped inside. The facility further failed to ensure fall interventions were in place for a resident with diagnoses including myocardial infarction, dementia, and bipolar disease who had multiple falls over time. The resident’s record contained repeated IDT notes listing interventions such as anti-roll backs and extended brakes on the wheelchair, nonskid strips by the bed, brightly colored tape on the call light, a bolster mattress, floor mats, and a bed in the lowest position, but survey observations found these interventions were not in place in the room or on the wheelchair on multiple occasions. The resident’s record also lacked a quarterly fall risk assessment that was due, and the last assessment on file was dated months earlier and indicated no history of falls.
