Summary

Provide appropriate pressure ulcer care and prevent new ulcers from developing was not met when a resident with a left foot brace and tight socks developed skin breakdown on both lower legs. Resident K was observed seated in a wheelchair with a plastic brace pressing against the back of his left calf and a sock that did not come above the calf. A red abrasion encircled his right calf, and the sock on his right foot was tight around the lower leg. During interview, the resident stated the abrasion was from his socks being too tight and that the brace sometimes pressed into the back of his leg and hurt because he could not get his socks up over his calf anymore. The resident’s record lacked documentation of the abrasion around his right leg and did not document that he wore an orthotic brace on his left foot. Nursing staff later noted a new skin tear and abrasion on the lower leg, with small bleeding and no drainage, and the wound nurse and NP were not aware of the new skin abrasion until the resident was assessed. During that assessment, a second abrasion/skin tear was noted under the left sock near the ankle, and the NP and wound nurse agreed the abrasion had been caused by the socks being too tight and that the brace needed to be padded or the skin protected to prevent further breakdown. A second resident, Resident F, had a physician’s order for a gel pressure reducing cushion to be placed in her wheelchair and a care plan intervention stating the gel cushion should be used while she was up in the chair. Despite this order and care plan, she was repeatedly observed in a regular wheelchair without a pressure reducing cushion during multiple observations over several days. Her record identified her as a long-term care resident with dementia and risk factors for impaired skin integrity, including decreased mobility, incontinence, chair fast at times, and impaired sensory perception.
