Failure to Update Care Plans for Falls and Wound Interventions

Summary

The facility failed to update care plans for two residents based on documented changes in condition and new interventions. For Resident 7, a long-term resident with diagnoses including myocardial infarction, dementia, and bipolar disease, Interdisciplinary Team notes documented falls on 9/9/24, 9/13/24, and 12/22/24 with new interventions added after each event, including antiroll backs and extended brakes on the wheelchair, nonskid strips next to the bed, brightly colored tape on the call light, a bolster mattress, and offering assistance to lie down after meals. However, the resident’s care plan reviewed on 9/19/25 only reflected a later fall care plan with interventions such as a low bed, fall mats, night shift assistance to the bathroom, OT evaluation, and reminders to call for assistance, and it did not include the interventions documented in the IDT notes. No other fall care plans were present in the record. For Resident 5, a resident with diagnoses including fracture of the left lower leg, type 2 diabetes, hypertension, multiple left rib fractures, major depressive disorder, and difficulty walking, the record showed orders for daily treatment of bilateral heel diabetic ulcers and heel protectors for both heels at all times when in bed. During observation, the resident was in bed with the left leg and heel on a flat pillow, and the heel protectors were in the windowsill. The current care plan for a documented pressure ulcer and pressure ulcer management did not include the intervention requiring heel protectors at all times when in bed. The MDS nurse stated she would address the care plan.
