Summary

The facility failed to implement care plans for the advanced directives of 2 residents reviewed for care plans. Resident 5 had diagnoses including fracture of the left lower leg, type 2 diabetes, hypertension, multiple left rib fractures, major depressive disorder, and difficulty walking, but her record lacked a care plan containing advanced directives. On 9/18/25, the MDS nurse indicated she added a care plan for the resident's advanced directives and full code status. Resident 21 was a long-term care resident in the facility for rehabilitation from a wound and had an active order for full code status, but her care plans were reviewed and her record lacked a care plan containing code status. On 9/18/25, the DON provided a care plan dated 9/18/25 indicating she had a full code status. The facility also failed to implement a care plan for Resident 7's behaviors. Resident 7 had diagnoses including myocardial infarction, dementia, and bipolar disease. Surveyors observed repeated room conditions including a strong urine odor, a bag of visibly soiled clothes on the floor, trash from suckers on the bedside table and floor, gnats on and around the trash and clothes, and food and drink items left in the room over multiple observations. The DON stated Resident 7 sometimes refused to let CNAs change him when his clothes were soiled, often would not let housekeeping or staff clean his room, would spit grape peels on the ground, and would leave trash from snacks in his room and on the floor. His care plans lacked interventions for these behaviors, including refusal to be changed, refusal of room cleaning, and collecting food and trash.
