Summary

The facility failed to ensure accurate resident assessments for 10 of 13 resident records reviewed. Surveyors found that multiple MDS assessments did not match the residents’ current diagnoses, physician orders, medication administration records, or observed status, including incorrect coding of medications, hospice services, and physical restraint use. For one resident with alcohol-induced dementia, anxiety, depression, and bipolar disorder, the quarterly MDS indicated use of an antipsychotic, antidepressant, hypnotic, opioid, anticoagulant, and antiplatelet medication, along with a completed GDR and a contraindication to GDR on the same date. However, current orders and MAR review showed no current antipsychotic, hypnotic, or anticoagulant medication, and the social services note stated only a contraindication to decrease Cymbalta due to neuropathy, with no medications approved for GDR. The MDS Coordinator stated Lamictal had been incorrectly identified as an antipsychotic and that hypnotic and anticoagulant coding should not have been entered. Several residents had MDS assessments that coded bed rails as physical restraints even though staff stated the rails were used as enablers or for mobility. Residents had no physician orders or care plans for restraints, and observations showed side rails in use while staff described them as not being restraints. Another resident’s MDS incorrectly indicated no hospice services despite an active hospice order and staff confirmation that hospice services were being received. Additional residents had medication coding errors, including missed anticonvulsant use and incorrect coding of antiplatelet, hypnotic, anticoagulant, and diuretic medications. The MDS Coordinator acknowledged multiple coding errors during interviews.
