Summary

The facility failed to ensure that opened food items were labeled and dated, expired food was discarded, and thermometers were properly maintained in cooled and frozen food storage areas in the kitchen. During observation of the walk-in refrigerator, a Ziploc bag containing pink sliced meat had illegible markings, a sealed bag of ham was dated 8/27/25, and a thermometer on an upper shelf read 16 degrees even though no items in the refrigerator appeared frozen solid or had visible frost. The Dietary Manager stated opened items should be used within 3 days or discarded and indicated the thermometer did not appear accurate, so it was replaced. In the walk-in freezer, no internal thermometer was observed, and the Dietary Manager stated the thermometer had been present there 3 days earlier. Several stacked trays of bowls of thick brown substance were covered with plastic wrap, but no label or date was visible on any bowl, wrapper, or tray; a container of mint chocolate chip ice cream also had no visible label or date, and a jar containing a reddish tan substance with 2025 written on the lid was stored in the freezer. The Dietary Manager stated the trays likely contained scooped chocolate ice cream prepared that morning but not directly observed, and said the ice cream and jar should have been labeled and dated. Additional observations found a plastic scoop in the sugar supply, loose rice in a tied bag of rice, no interior thermometer in the reach-in refrigerator, and unlabeled items in a unit refrigerator including salad dressing, pasta, an open water bottle, and a wet towel on the bottom shelf. The DON stated all residents in the facility were served food prepared in the kitchen.
