Medication Given Outside Ordered Blood Pressure Parameters

Aperion Care KokomoKokomo, Indiana Survey Completed on 08-15-2025

Summary

The facility failed to follow a physician’s order for metoprolol tartrate for Resident 50, whose diagnoses included atrial fibrillation, cardiomyopathy, chronic systolic congestive heart failure, hypertension, acute embolism and thrombosis of the deep veins, and a history of pulmonary embolism. The order, dated 7/8/25, directed that the medication be given by mouth twice daily for hypertension and held if systolic blood pressure was less than 120 or heart rate was less than 60. Review of the MAR showed metoprolol was administered outside the ordered parameters on multiple occasions, including when systolic blood pressure readings were 109, 111, 111, 104, 108, 119, and 116. A care plan for hypertension or for use of a medication to treat high blood pressure was not located in the clinical record. During interview, QMA 2 stated that if vital signs were outside medication parameters, the medication would be held and the supervising nurse informed, and the DON stated medications should not be given if vital signs were outside the physician’s ordered parameters.

D
F0684 F684: Provide appropriate treatment and care according to orders, resident’s preferences and goals.
Short Summary

Failure to provide ordered wound care and aspiration precautions: A resident returned from a dermatology procedure with biopsy-site dressings and the chart lacked documentation of assessment or wound care, while the dressings remained in place and undated during observations. The same resident also had dysphagia with a FEES showing silent penetration with straw sips and an order for no straws, yet was observed drinking water from a cup with a straw; the SLP and DON confirmed the no-straw precaution and expectation to follow physician orders.
