Summary

The facility failed to develop and implement effective interventions and monitoring for a resident with dementia who displayed known wandering and physical aggression. Resident 53 had diagnoses including dementia with behavioral disturbance, major depressive disorder, anxiety disorder, and insomnia. His record showed severe cognitive impairment, daily wandering, disorganized thinking, incoherent conversation, and unpredictability. Orders included a WanderGuard to the left ankle, behavior monitoring each shift, and care plan interventions for wandering, elopement risk, and aggressive behaviors.
