Food Temperature Monitoring and Dishwashing Sanitation Failures

Beaumont Rehabilitation And Healthcare CenterAnderson, Indiana Survey Completed on 09-19-2025

Summary

The facility failed to prepare, serve, and distribute food under safe sanitary conditions related to food temperatures at the time of service. During a kitchen tour, the three-compartment sink was observed with food particles in the wash sink, utensils in the rinse sink, and utensils and pans in the sanitizing sink. The Dietary Manager tested the sanitizing sink and the test strip registered zero sanitizing agent. The Dietary Manager stated the water was too cold and would not activate the sanitizer when cold, and that the water temperature booster had not been working properly. A later observation again found food particles in the wash sink, utensils in the rinse sink, and utensils and pans in the sanitizing sink, and the sanitizing solution again registered zero sanitizing agent. Two roasting pans stored to dry were also observed with water droplets on them. The facility also failed to ensure food temperatures were taken and documented at the point of service on the steam table. During lunch meal service, a cook began serving meal trays without taking temperatures of the food items on the steam table and served 11 meal trays without taking any food temperatures. The cook stated he had taken temperatures earlier at the oven/stove before the food was placed on the steam table and held until service, and that he did not record the temperatures on the log until after the meal was done. Review of the Daily Food Temperature Sheets showed several meals with missing documented food temperatures, including dinner, breakfast, and lunch entries on multiple dates. The Dietary Manager stated all 116 residents received meals prepared in the facility kitchen.

A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
