Failure to Maintain Enhanced Barrier Precautions for a Resident With a G-Tube

Summary

The facility failed to follow Enhanced Barrier Precautions for one resident who had a G-tube and diagnoses including COPD, protein-calorie malnutrition, and dysphagia. The resident’s record showed a BIMS score of 14 and that he required partial/moderate assistance with eating. His care plan identified that he required tube feeding when he ate 50% or less of meals and that he was at risk for complications. The order summary also documented orders related to flushing and verifying placement of the G-tube, along with an order to initiate EBP that was later shown as discontinued and then re-initiated. Observation and record review showed that the resident’s room did not have EBP signage, PPE outside the doorway, or isolation bins inside the room on multiple observations. On one observation, a CNA and another staff member took the resident to the restroom without donning a gown or other PPE. On another observation, an LPN performed a G-tube flush after first donning a gown, gloves, and mask, then measured the tube and flushed it with 60 cc of water. The DON stated that the resident had been taken off EBP after the surgical wound on his hip healed, and he was unsure whether the resident should still have been on EBP because of the G-tube. The facility policy stated that EBP applies to residents with wounds and/or indwelling medical devices, including feeding tubes, regardless of MDRO colonization status, and that signs should alert staff and visitors to the precautions required. The policy also stated EBP is intended to remain in place for the duration of the resident’s stay or until the wound resolves or the device is discontinued. The DON stated he would put the resident back on EBP, and later observation showed EBP signage and PPE available at the resident’s door.
