Summary

The facility failed to maintain an effective pest control program as required by its own policy and procedure, resulting in the presence of cockroaches in the kitchen, adjacent meal serving/steamtable area, and resident rooms. Observations included remnants and carcasses of cockroaches on glue boards and counters in the kitchen and serving areas. Staff interviews confirmed ongoing awareness of cockroach activity in these areas, with both maintenance and dietary staff acknowledging the presence of live and dead cockroaches. Invoices from the pest control company documented recurring cockroach issues in the kitchen and public areas over several months.
