Summary
The facility failed to ensure that care plans were revised as needed for a resident with multiple diagnoses, including sleep apnea and chronic obstructive pulmonary disease. A physician's order directed staff to provide continuous oxygen at 2 LPM via nasal cannula, and the resident's care plan reflected this order.
