CNAs stated that facility-employed CNAs performed nail care, while agency CNAs did not assist with nail care for safety reasons; another CNA stated nurses clipped nails and CNAs cleaned and filed them. Resident #90 was not diagnosed with diabetes, yet there was no documentation of attempted nail care during the week leading up to the last observation, even though she had refused showers on two scheduled shower days and her bath schedule was twice weekly. Her care plan addressed ADL assistance, bathing, and resistiveness to care, but the record did not show nail care being addressed during the period reviewed.
