Summary

The facility failed to protect multiple residents from abuse by other residents, as evidenced by several documented incidents involving physical altercations. Residents with significant psychiatric and cognitive impairments, including schizophrenia, bipolar disorder, dementia, and anxiety, were involved in repeated episodes of aggression.
