Summary

The facility failed to update a resident's care plan after being notified of unauthorized use of the resident's credit card by a nurse aide. The resident, admitted for short-term rehabilitation with diagnoses including diabetes, anxiety, and heart failure, was assessed as having no cognitive impairment and required assistance with personal care.
