Summary
The facility failed to develop a care plan to address the treatment of a rash for one of five residents. The resident was admitted with diagnoses including neuropathy and chronic obstructive pulmonary disease (COPD). A Change of Condition Note documented that the resident had fragile skin and an erythematous maculopapular rash scattered over the trunk, back, and all extremities. Review of the resident's care plans showed there was no care plan in place to address the rash. During an interview, the DON confirmed the absence of a care plan for the rash.
