Summary

The facility failed to ensure a safe discharge for one resident who needed assistance with ADLs and was discharged to an ILF that did not provide assistance. The resident was admitted with sepsis and infection/inflammatory reaction related to a left hip prosthesis. His care plan stated he wanted to return to the community safely, and facility staff were expected to identify a safe discharge location, provide community resource information and assistance, and complete a welfare check after discharge. Records showed the resident used a wheelchair, could ambulate only with a front wheeled walker and contact guard assist, needed help with meals, and required one-person physical assist with bathing. The ECA stated ILFs provide lodging, not assistance with daily needs, and denied telling the SSD the ILF would provide ongoing help. The ILF manager stated the resident arrived unable to walk, the facility was not wheelchair accessible, and the bathroom was not accessible; he said he would not have accepted the resident if he had known he could not walk. The resident stated he asked whether the discharge location could assist him, trusted the SSD when told it would, and reported he did not eat between discharge and hospital arrival because he could not cook and needed help setting up his food.
