Summary

Facility staff failed to ensure that Resident 67 was screened for a need for trauma-informed care upon admission. Resident 67 was admitted with diagnoses including hemiplegia, hemiparesis following a cerebral infarction, and PTSD. His care plans documented behavioral problems with verbal outbursts, agitation, and anxiety, with staff directed to remove him from triggering environments, place him in a calm and quiet area with supervision, and identify and attempt to remove anxiety triggers. During observation, Resident 67 was pacing in his room with a seated walker and stated that he did not feel safe in the facility. He said he felt threatened by another resident and wished he had a gun to protect himself. Staff interviews indicated that he was often assigned to an LPN who was unsure what triggered his behavior, and described incidents in which loud singing caused him to slam his door and scream, and another where he believed he was being followed by another resident. The SSD stated the facility used a Trauma Informed Care screening form on admission and confirmed that the screening was not completed for Resident 67, despite understanding it would have helped formulate his care plan for trigger mitigation or elimination.
