Summary

Nursing staff failed to develop a care plan for Resident 53's oxygen therapy. Resident 53 was admitted to the facility on 8/23/25 with diagnoses that included COPD, pleural effusion, and atelectasis. The physician order report showed an order for supplemental oxygen at 2-3 LPM and for licensed nurses to check the resident's pulse at least once during every shift starting on 9/18/25. A review of Resident 53's care plans dated 9/18/25 to 9/25/25 did not include a care plan for oxygen therapy. During an observation on 12/2/25 at 10:30 a.m., Resident 53 was sitting up in bed awake and alert, wearing a nasal cannula, and the oxygen concentrator was set at 2 liters of humidified oxygen. During an interview on 12/4/25 at 4 p.m., LN 6 confirmed there was no care plan for Resident 53's oxygen therapy. The facility's Care Plan Essentials policy stated that respiratory information is included in the resident-specific care plan and that a new CPE is initiated when an update in any section is indicated.
