Summary

The facility failed to follow monthly pharmacist medication regimen review (MRR) recommendations for two residents. Resident 18, admitted with psychotic disorder with hallucinations and recurrent major depressive disorder with severe psychotic symptoms, had an order for olanzapine 15 mg at bedtime for auditory hallucinations. The record showed no episodes of auditory hallucinations in September 2025, two episodes in October 2025, and seven episodes in November 2025. The November 2025 MRR included a pharmacist recommendation for a gradual dose reduction (GDR) of olanzapine, but the recommendation was left unmarked with no action documented. During interview and record review, the DON confirmed the resident started olanzapine in June 2025, a GDR was not attempted, and the pharmacist recommendation was not addressed. Resident 86, admitted with depression and anxiety, had a lorazepam 0.5 mg PRN order for anxiety manifested by restlessness and inability to relax, started without a stop date, and a sertraline 50 mg daily order for depression started without a manifestation. The pharmacist’s MRR noted the lorazepam PRN order needed a 14-day stop date or provider documentation for extended therapy, but there was no physician response. A separate MRR noted sertraline needed a manifestation added, and there was no physician response. The DON confirmed the lorazepam PRN order had no end date beyond 14 days and stated sertraline should have a manifestation listed in the order.
