The facility failed to ensure two residents were free from unnecessary psychotropic medications. The consultant pharmacist reviewed the order in November 2025 and recommended adding a 14-day stop date or documenting the need for extended therapy, but there was no physician response. The same resident also had sertraline 50 mg daily for depression, started on 4/28/25 without a manifestation. The consultant pharmacist recommended adding a manifestation to the order, and the record again showed no physician response. The DON confirmed the lorazepam PRN order should have had an end date after 14 days and stated sertraline should have a manifestation listed in the order.
