Wound Care Not Completed Per Physician Orders
Summary
The facility failed to ensure that Resident #52’s surgical wound care was provided in accordance with physician orders and the resident’s care plan. Resident #52 was a cognitively intact male with a history of amputation, chronic osteomyelitis, lack of coordination, and unsteadiness on feet. His care plan identified a left forefoot amputation related to gangrene and a post-surgical wound of the left distal foot, with interventions to monitor the wound daily and apply lodosorb, gauze roll, and kerlix. Physician orders directed cleansing the left distal foot wound with normal saline, applying iodosorb, covering with alginate, and wrapping with kerlix on a Monday/Wednesday/Friday schedule. On observation, Resident #52’s right foot had no dressing even though there was an order for treatment, and the resident stated staff were no longer wrapping it because it had healed. The left foot dressing was observed to be loosely wrapped around the foot and ankle, with a deep red area at the toes, gauze not in place, and the wrap not supporting the wound. The resident stated the dressing had last been changed two days earlier and had not been changed since. Later observation showed the left foot fully wrapped, but the toe area had soiled through the dressing and onto the top sheet. The wound care nurse stated he had only been in the position for two days, was responsible for wound care, and that charge nurses would complete care if he was not in the building. He stated the dressing on the left foot had last been changed the prior day and that the way it was dressed when he saw it was not acceptable. He also stated staff initials and the date must be identified on the wound after care was provided. The ADON and DON both stated wound care was expected to be completed according to physician orders and that staff were to initial and date the dressing after treatment; they also stated failure to do so placed residents at risk of infection and missed treatments.
