Call Lights Not Kept Within Reach
Summary
The facility failed to ensure that two residents had their call buttons within reach. Resident #136 was admitted and readmitted with diagnoses including acute and chronic respiratory failure, unspecified dementia, heart failure, and encounter for attention to tracheostomy. Her MDS reflected a BIMS score of 13, indicating intact cognition, and her care plan identified her as at risk for falls with an intervention to ensure her call light was within reach and to encourage use of it for assistance as needed. During observation, she was found lying in bed asleep with the call light on the floor under the bed and not within reach. When interviewed later, she stated her cord was where she could reach it most of the time, but the call light was still observed on the floor. A CNA stated all call lights must be within reach and was not aware hers was not, then immediately went to the room to check it. Resident #112 was admitted with diagnoses including bipolar disorder with psychotic features, essential hypertension, and a history of traumatic brain injury. Her MDS reflected a BIMS score of 0, indicating severely impaired cognition, with disorganized or incoherent thinking and unclear or illogical flow of ideas. Her care plan identified her as at risk for falls related to limited mobility, incontinence, and impaired cognition, and included interventions to anticipate and meet her needs and keep personal items within easy reach, but did not include keeping the call light within reach. During observation, she was resting in bed with the call light attached to the fitted sheet on the mattress and the cord hanging down to the floor under the bed, leaving the button out of reach. When interviewed, she stated she did not know what the call button was. Staff interviews confirmed that call lights were expected to be within reach and answered promptly. An LVN stated call lights were to be within reach, answered immediately or within 5 to 10 minutes, and that residents unable to push the button could be provided a push pad. Another LVN and a CNA gave similar statements that call lights should be within reach and answered as soon as possible. The ADM also stated she expected call lights to be within reach and answered. The facility policy for resident rights stated residents have a right to a safe, clean, comfortable, and homelike environment, including receiving treatment and support for daily living safely.
