Unqualified oversight of food and nutrition services
Summary
The facility failed to have qualified, full-time oversight of Food and Nutrition Services in accordance with State requirements. The deficiency was based on interview and document review showing that the Registered Dietitian did not perform the day-to-day management and supervision of dietetic services described in the job description, and the staff placed in charge of supervising the kitchen were not qualified under California requirements. During an observation and interview, the Environmental Director and the Assistant Dietary Supervisor were found in the supervisor office in the kitchen. The Environmental Director stated she was the kitchen supervisor, but also said she was responsible for Central Supply, Housekeeping, and Maintenance. The Assistant Dietary Supervisor stated she helped supervise the kitchen. The Administrator later stated the Registered Dietitian did not oversee the day-to-day operation of the kitchen and that the Assistant Dietary Supervisor oversaw it, but was not qualified. The Registered Dietitian stated she handled clinical nutrition only and completed a monthly sanitation checklist in the kitchen. The Environmental Director stated she had a supervisor role in the kitchen, including staff scheduling, contacting vendors, ordering equipment, and overseeing needed items, and that she and the Assistant Dietary Supervisor evaluated kitchen staff quality of work. The Administrator also stated the Registered Dietitian was in charge of all Food and Nutrition Services, but later acknowledged that the Assistant Dietary Supervisor and Environmental Director did not meet any of the qualification pathways listed in HSC 1265.4(b). The facility organization chart showed the Dietary Services Supervisor reported to the Registered Dietitian and Administrator, and dietary staff reported to the Dietary Services Supervisor, with handwritten names for the Dietary Supervisor and Registered Dietitian positions.
