Incomplete Care Planning for Schizophrenia and Bed Rail Use

Mission De La CasaSan Jose, California Survey Completed on 08-21-2025

Summary

The facility failed to develop individualized, resident-centered care plans for multiple residents with identified needs. Resident 15 had a diagnosis of schizophrenia on the facesheet dated 8/14/25, but during a 8/15/25 review with the DON, no care plan for schizophrenia was found. The DON stated that Resident 15 should have had one. The facility policy titled "Comprehensive Care Plans-Timing" stated that each resident has a person-centered, comprehensive care plan developed, reviewed, and revised by the interdisciplinary team, including the resident representative if applicable. The facility also failed to develop care plans for bed rail use for Residents 22 and 64. Resident 22 was observed on 8/12/25 with two bilateral bedrails attached to the bed, and RN G confirmed on 8/18/25 that no person-centered care plan had been developed and implemented for bedrail use as a positioning device. Resident 64 was observed on 8/12/25 with bilateral bedrails up, and RN G confirmed on 8/18/25 that no person-centered care plan had been developed and implemented for Resident 64's bedrail use, and that one should have been. For Residents 55, 92, 78, 70, 89, 60, and 67, the positioning device care plans included an intervention to re-evaluate resident safety and use of bed rails quarterly, but this was not implemented. Resident 55's latest bed rail use assessment was dated 6/18/2021, and MDSN K confirmed the quarterly re-assessment had not been done. Resident 60's latest assessment was dated 12/5/2024, and there was no documentation that alternatives were offered or attempted prior to bed rail use. Resident 67's latest assessment was dated 11/11/2024, Resident 70's was dated 1/29/2025, Resident 89's was dated 12/9/2024, Resident 78's was dated 4/10/2024, and Resident 92's was dated 9/27/2024; in each case, MDSN K confirmed the quarterly re-evaluation intervention was not implemented. For Resident 136, the positioning device care plan was initiated on 7/16/2024 and resolved on 4/15/2025, but MDSN K stated there should have been a care plan developed related to continued bed rail use.

Failure to Care Plan External Catheter Urinary Wicking System
D
F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

Failure to care plan an external catheter urinary wicking system: A resident with anxiety, chronic pain, scoliosis, and end stage HF was admitted cognitively intact but dependent for toileting and personal hygiene and always incontinent. His care plan addressed incontinence care, but it did not include the external catheter system, who would reapply it, or when the collection canister would be emptied. The resident said he needed help with setup and reapplication, urine containers were observed on the floor with one full of dark yellow urine, and the DON stated the system should have been addressed on the care plan.

Short Summary

Incomplete and Outdated Person-Centered Care Plans: A resident’s care plan did not include full code status even though the chart and orders documented full code, and another resident’s care plan was not revised after recent behaviors led to a psych assessment documenting instability and directing redirection. The records showed significant medical and cognitive diagnoses, but the care plans did not fully reflect the residents’ current needs and status.

Incomplete care planning and documentation for dialysis nutrition and catheter self-care
D
F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

The facility failed to develop and implement complete care plans for two residents. One resident on dialysis had a care plan for ordered diet and meal intake monitoring, but multiple meal percentages were not documented after dialysis meals. Another resident with a suprapubic catheter was observed with an exposed, uncapped attachment nozzle, and the care plan did not include the resident’s self-care of the catheter. Staff and the DON confirmed the resident ate after dialysis and that the catheter tip should be covered when switched.
