Summary

The facility failed to ensure infection control measures were followed when two employees from a consulting cardiologist's office entered Resident 59's room and provided care without using the required Enhanced Barrier Precautions. Resident 59 was observed in bed, alert, with his upper body half exposed while Cardiac Sonographer 2 sat less than two to three feet from the bed and Cardiac Sonographer 1 stood next to her. Both employees wore gloves but did not wear gowns, and medical equipment was present in the room while they performed an ultrasound on Resident 59. During a later observation and interview, the two employees stated they were performing an ECG on Resident 59 and had only used gloves during the test. Resident 59's room had a sign indicating both he and his roommate were on Enhanced Barrier Precautions, and PPE supplies including gown, mask, and gloves were stored by the door. The Infection Prevention Nurse stated Resident 59 was on EBP because he had a foley catheter and that staff should wear gown, glove, and mask when providing patient care. The nurse also stated the consulting employees should have followed the proper EBP when they entered the room. Resident 59's record showed a physician's order for Enhanced Barrier Precautions during high-contact resident care activities due to an indwelling foley catheter, and the care plan directed staff to use gown and gloves during high-contact resident care activities.
