Summary

The facility failed to ensure that a left elbow skin discoloration identified on a resident was reported and referred to the physician in a timely manner for treatment orders. The resident was admitted with dementia and the history and physical indicated she could make needs known but could not make medical decisions. Her physician’s order required monitoring every shift for bleeding, bruising, and skin discoloration related to Plavix use, and the care plan directed staff to monitor for bruising or bleeding and report abnormal findings to the physician. On September 9, 2025, the resident was observed in bed with a purplish skin discoloration on her left elbow; the skin was intact and there was no redness or swelling around the area. In a concurrent interview, the resident stated the discoloration had been present for days and did not hurt, and she believed it may have occurred during repositioning or when her elbow hit the side table. The shower skin sheet for that day indicated the CNA documented no apparent skin issues, and the eMAR showed licensed nurses monitored for bleeding, bruising, and skin discoloration every shift but did not identify a new discoloration during that period. Record review and interviews showed there was no documented evidence that the elbow discoloration was reported, assessed, or referred to the physician for treatment orders. The day shift LVN stated she did not receive a report from the CNA or another nurse about the discoloration. The TN stated she had not received a report that the resident had a skin discoloration on her left elbow and confirmed there was no current treatment order or care plan for it. The DON stated the discoloration should have been identified and referred to the physician immediately, and that staff’s failure to identify, assess, determine the cause, and notify the physician resulted in a delay in treatment.
