Summary

The facility failed to ensure that the Direct Care Service Hours Per Patient Day (DHPPD) information, which includes the updated census and number of staff on duty, was posted daily as required. During an observation and interview at Nursing Station 1, the DHPPD posted was found to be three days old, and the Director of Staff Development acknowledged that the information was not current and should be updated daily.
