Summary

The facility did not provide pharmaceutical services to ensure proper dispensing of medications, did not keep drug records in order, and did not maintain and periodically reconcile controlled drugs. On 08/26/2025, surveyors reviewed the narcotic count logs for unit 2 South and found multiple dates in both count books where there were not two signatures by licensed nurses, despite the facility policy requiring a physical inventory of all controlled substances at each shift change or when keys are transferred. Surveyors also observed the medication room on 1 East/West and found the medication refrigerator log had not been updated since July 2025, and LPN-Q was unable to locate a thermometer inside the refrigerator. The DON stated that all medication refrigerators should have a thermometer and that temperatures should be logged, and also reported that maintenance had been contacted weeks earlier about a 2 South refrigerator that would not close because of ice buildup. The facility also failed to ensure medication administration for a resident with rib fractures. R120 was admitted with a diagnosis including fracture of the ribs and had an order for Lidocaine 5% patch to be applied every morning and removed at bedtime. On 08/25/2025, an LPN observed a Lidocaine patch dated 8/22/25 still on R120's right rib area when preparing to apply a new patch. The old patch was removed and replaced, and R120 stated she wondered why she had not been getting the patch every day because it helped with rib fracture pain. Review of the MAR showed the patch was signed out as placed in the morning and removed at bedtime from 8/22/25 to 8/22/25 bedtime, even though R120 was admitted in the evening on 8/22/25 and should have had the hospital patch removed that night and then received new patches on 8/23/25 and 8/24/25. No explanation was provided for why the Lidocaine orders were not followed.
