Enteral Feeding Tubes Not Properly Labeled or Documented

Summary

The facility failed to ensure appropriate treatment and services for residents with enteral feeding tubes. For one resident with dysphagia and a PEG tube, the physician orders included bolus Nepro after meals if less than 50% of the meal was consumed, free water flushes, and residual checks, but the physician order summary did not include PEG site care and the TAR did not document PEG site care since admission. During observation, the resident was eating breakfast and was not connected to the tube feeding pump, which had a 1-liter bag of clear liquid and a 1-liter bag of brown liquid hanging in the room without identifying labels; the brown liquid bag was dated 8/6 with no identifying labeling, and the clear liquid bag had no identifying labeling. The ADON stated PEG site care was done daily but did not know what happened to the physician orders or TAR documentation and discarded the fluid-filled bags without knowing why they were there. For another resident with paraplegia, Parkinson's disease, cognitive communication deficit, dysphagia, and a G-tube, the care plan identified enteral feeding as necessary to maintain nutritional and hydration status. On observation, the resident had a tube feeding pump running with a 1-liter bag of clear liquid and a 1-liter bag of brown liquid, with 800 mL of brown liquid in one bag, and the tubing was connected to the gastrostomy tube. The hanging bags and tubing initially had no date, time, label, or description of the contents. On a later observation, the bags were dated with black marker, but the liquids still were not labeled and the tubing still had no date or label. The NHA and DON stated nursing staff should label the hanging liquids and the line connecting the bags to the machine, and the NHA stated it was the expectation that staff label, date, and initial the tube feeding bags and line.
