Unqualified Dietary Manager

Edenbrook Of Green BayGreen Bay, Wisconsin Survey Completed on 08-27-2025

Summary

The facility did not designate a person to serve as the director of food and nutrition services who met the required qualifications of being a certified dietary manager, a certified food service manager, holding a national certification for food service management and safety from a national certifying body, or having an associate's or higher degree in food service management or hospitality. This deficiency was identified through staff interview and record review and was noted as having the potential to affect all 70 residents in the facility. During review of the dietary manager's credentials, the surveyor received documentation showing the dietary manager did not pass the dietary manager certification test and was not certified. The nursing home administrator stated the dietary manager had not passed the test, could not retake it for 90 days, and that no waiver had been filed with the State Agency for the position. The dietary manager confirmed failing the test and stated there was no food service manager certification or relevant degree. The registered dietitian was aware of the lack of required certification and stated they were working with the dietary manager to study for the next test.
