Incorrect PPE Use and Hand Hygiene Lapses

Summary

The facility did not establish an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections. Surveyors observed two staff members incorrectly wearing PPE on the COVID-19 positive hallway. On the 100 hallway, fire doors were closed and signs were posted on both doors stating staff must wear a mask. The NHA stated the 100 hallway had COVID-19 positive residents. Surveyors observed a CNA entering a room wearing an N95 mask over a surgical mask, and the CNA stated it was okay to wear both masks because both were removed upon exiting the room. Later the same day, another CNA on the same hallway was also observed wearing an N95 mask over a surgical mask and stated that wearing both masks at the same time was not correct. The DON stated it was not the facility’s policy or standard of practice to wear an N95 over a surgical mask. The facility also failed to follow its hand hygiene policy during medication administration. The policy stated gloves do not replace hand hygiene and that staff should perform hand hygiene prior to donning gloves and immediately after removing gloves. Surveyors observed an RN apply gloves, prepare insulin supplies, enter a resident’s room, administer insulin, remove her gloves, and then put on a new pair of gloves without using hand gel or washing her hands. The RN stated she used gloves to prep medications and indicated she did not perform hand hygiene between changing gloves because she did not touch anything. During interview, an LPN described when hand hygiene should be performed, including before and after using gloves, and the DON stated hand hygiene is to be performed after removing gloves and before doing anything else.
