Unsafe Food Storage and Preparation Practices

Summary

Food was not stored, prepared, distributed, and served in accordance with professional food service safety standards. Surveyor observations in the kitchen found leftover food items that were not properly covered, labeled, or dated, along with cleanliness concerns in the main kitchen. Food was observed stored on the floor in the dry storage room, walk-in refrigerator, and walk-in freezer. Surveyor also observed staff touching the inside surface of a food container with a bare hand and staff preparing food without using a recipe. During the kitchen tour, prepared salads in the walk-in refrigerator were partially covered with parchment paper, had brown lettuce, and were not dated with a preparation date or use-by date. Several pitchers of purple and pink liquid were also not dated. In the dry storage room, personal belongings, snack boxes, and debris were on the floor. In the walk-in freezer, empty boxes, packaging debris, and wrapped food were on the floor; when questioned, the dietary manager picked up a package of breadsticks from the floor and placed it on a shelf before discarding it after surveyor questioning. The main kitchen floor had food debris, wrappers, butter packet tops, bread ties, food spatter on the stove and tray line, and broken eggshells on the flat top stove. Surveyor observed dietary staff preparing lunch without beard restraints, including the dietary manager and dietary aides. One dietary aide stated he had facial hair and should have been wearing a beard restraint, and later walked in and out of the kitchen without one. The dietary manager stated the facility did not have beard restraints available and staff were wearing masks instead. During tray line preparation, mashed potatoes were corrected by adding unmeasured hot water and an unmeasured amount of potato flakes. The dietary manager later acknowledged that the water and potato flakes should have been measured, and that staff should only touch the outside of containers or handles. The nursing home administrator stated the kitchen should be clean, food should not be stored on the floor, food should be covered and dated, staff should wear appropriate facial hair restraints, recipes should be followed, and proper handling should be done of dishes, and said the observed conditions were unacceptable.
