Summary
The facility failed to maintain an effective infection prevention and control program, as evidenced by an incident during meal service. A registered nurse was observed dropping a cup lid onto the dining room floor, picking it up, and placing it on the counter before filling the cup with ice and a drink. The cup and lid were then handed to a nurse aide, who placed the lid onto the cup and served it to a resident.
