Summary

A resident with a history of schizophrenia, major depressive disorder, anxiety disorder, and prior aggression towards others was involved in a physical altercation with another resident. The care plan for this resident identified a potential for behavioral issues and aggression. On the date of the incident, the resident was observed sitting in the dining area when another resident walked by. The aggressive resident began calling the other resident names and then struck them in the chest multiple times. The incident was witnessed and subsequently reported to the state agency as a facility reported incident (FRI).
