Summary

The facility failed to provide care and services to assist a resident with activities of daily living, specifically in maintaining personal hygiene through scheduled showers. Record review showed that a female resident with severe cognitive impairment, dementia, generalized anxiety disorder, and major depressive disorder was dependent on staff for bathing and other ADLs.
