Summary

The facility failed to ensure that the posted Daily Staffing information was accurate and up to date for two consecutive days. Observations at both the North Wing and South Wing nurse's stations on multiple occasions revealed that the staffing information displayed was for the previous day rather than the current date. Specifically, on 8/26/2025, the posted information was dated 8/25/2025, and on 8/27/2025, the posted information was dated 8/26/2025. During interviews, the Assistant Director of Nursing (ADON), who was responsible for posting the staffing information, confirmed that the postings were not current and acknowledged that the information should be updated first thing in the morning.
