During interview, Resident 5 stated she could not access the grievance information across from the second-floor nursing station because it was too high to read or see from her wheelchair, and she could not read the names, addresses, and telephone numbers of the pertinent State agency and advocacy groups. An observation of the area also showed the posted information was obstructed by a shredding box, preventing a person in a wheelchair from moving closer to the posting. The Nursing Home Administrator acknowledged that the postings were arranged in a manner that created obstructed access for residents who used wheelchairs.

This deficiency had the potential to affect all 89 residents identified on the census list provided by the Administrator on 04/29/26, as residents and their representatives would not be aware of how to contact the Ombudsman about concerns they have.
