The facility failed to ensure that medication regimens were free from potentially unnecessary psychotropic medications for four residents. Review of facility policy showed that psychotropic medications are to be clinically indicated and necessary, and that medication regimen reviews are to be completed monthly with pharmacist recommendations reviewed and addressed by the DON and attending physician. Survey review found that the physician responses to several pharmacist recommendations were not documented in the clinical records for the residents involved. For one resident with dementia, bipolar disorder, and depression, the record showed bupropion ER 300 mg daily and Lamictal 25 mg twice daily for bipolar disorder. A psychiatric note recommended a gradual dose reduction for bupropion from 300 mg to 150 mg, but the record did not show that the physician addressed and signed the pharmacist review or that the recommended GDR was completed. For another resident with adjustment disorder, pseudobulbar affect, and vascular dementia, Xanax 0.25 mg every 12 hours as needed was ordered without a 14-day stop date or documented physician rationale for use beyond 14 days, and the resident received the PRN Xanax six times in September 2025. For a third resident with dementia, heart failure, and diabetes, the record did not include a completed physician response to the pharmacist’s May 2025 recommendations. The pharmacist noted hydroxyzine 10 mg at bedtime as a psychotropic medication and stated that a GDR must be attempted unless clinically contraindicated, but the physician did not address and sign the review. For a fourth resident with dementia, heart failure, and high blood pressure, the record also lacked a written physician response to the pharmacist’s July 2025 recommendations. The pharmacist identified Buspar 7.5 mg twice daily as a psychotropic medication and noted that a GDR must be attempted unless clinically contraindicated, but the physician did not address and sign the review.

R10 received brexpiprazole 1.5 mg twice daily for agitation related to dementia and haloperidol 2 mg daily for delusions related to unspecified dementia, but the EHR did not show evidence of side effect monitoring for these antipsychotics. R41 had severe cognitive impairment and diagnoses including non-traumatic brain dysfunction, hypertension, PVD, hyperlipidemia, arthritis, Alzheimer's disease, stroke, and non-Alzheimer's dementia. R41 received risperidone 0.25 mg three times daily for hallucinations, and the EHR did not show evidence of side effect monitoring. R91 had severe cognitive impairment and schizophrenia and received olanzapine and risperidone for paranoid schizophrenia, but the EHR did not show evidence of side effect monitoring. The record also showed failures to obtain and document orthostatic blood pressures as ordered for R10, R41, and R91. For R10, monthly orthostatic blood pressure documentation contained identical readings across lying, sitting, and standing positions on multiple dates. For R41, some monthly orthostatic blood pressure checks were documented as refused, with no evidence the monitoring was attempted again later, and other entries showed identical readings in all positions. For R91, one monthly entry lacked standing blood pressure and pulse documentation, and another showed identical readings in all positions. Staff interviews confirmed orthostatic blood pressures should be completed monthly in lying, sitting, and standing positions, that identical readings in all positions were not expected, and that side effect monitoring for psychotropic medications was not yet in place.
PRN Lorazepam Orders Lacked Required Stop Dates

Administrative Nurse D confirmed on 06/03/2026 that this PRN lorazepam order did not have the required stop date, consistent with the facility policy limiting PRN psychotropic medications to no more than 14 days unless extended with documented rationale and a specific duration.
Unnecessary Antipsychotic Use Without Documented Indication

During interview, the LPN/Resident Care Manager stated the facility’s process was to first rule out infection, environmental factors, and pain, then refer to mental health, and stated the Seroquel order did not meet expectations. The DON also stated the resident should have indications for antipsychotic use and that this did not meet expectation.
