Summary
The facility failed to provide a resident and/or the resident's representative with a reconciliation of all pre- and post-discharge medications at the time of a planned discharge. Clinical record review showed that the resident was discharged without documented evidence that this medication reconciliation was given. Additionally, the resident's representative specifically requested the medication reconciliation after the discharge, but it was not provided at that time.
