The physician order dated 06/28/25 showed Ativan 0.5 mg by mouth every six hours PRN for anxiety with no stop date, and the MAR showed the resident received the PRN Ativan repeatedly in June, July, and August 2025. The consultant pharmacist review dated 07/15/25 noted no apparent medication irregularities, and the Administrator verified on 08/08/25 that the PRN Ativan order had no stop date. Resident #107 was admitted on 11/30/22 with diagnoses including end stage renal disease, dependence on renal dialysis, congestive heart failure, type two diabetes mellitus, Alzheimer's disease, dementia, and anxiety disorder. The August 2025 physician orders included Ativan 1 mg by mouth every eight hours PRN for anxiety with a start date of 07/02/25 and no stop date. The July 2025 MAR showed the resident received the PRN Ativan on most days after the start date, and the August 2025 MAR showed additional administrations on 08/01/25 through 08/05/25. The DON verified on 08/05/25 that the PRN Ativan order had no stop date.

The resident’s RP stated the resident’s sign of restlessness was isolating himself and reported that the resident texted her about having anxiety from 10:30 AM until 7 PM on 6/30/2026. The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
