Summary

The facility failed to update care plans for residents who smoked cigarettes, affecting two of four residents reviewed for smoking. Resident #21 was admitted with diagnoses including multiple sclerosis, subdural hematoma, anxiety disorder, anti-social disorder, and spondylosis. The MDS dated 07/02/25 showed mild cognitive impairment. The AD confirmed the resident was on oxygen and needed staff assistance to remove tubing before smoking, light cigarettes, and hold the cigarettes because of a hand tremor, but the smoking care plan had not been updated with interventions for safe smoking. The facility policy stated residents would be assessed for their ability to smoke safely and that this information would be included in the care plan.
