Summary

A resident with diagnoses including heart disease, depression, seizures, and intellectual disabilities was admitted on 07/11/23 and had impaired cognition, as evidenced by a BIMS score of 7 out of 15. The resident's quarterly MDS assessment indicated a need for moderate to dependent assistance with activities of daily living (ADLs), including personal hygiene and shaving. The care plan dated 06/06/24 specified that staff assistance was required for personal hygiene tasks. Observations on two consecutive days revealed the resident had noticeable facial hair on the upper lip and chin while resting in bed and during breakfast.
