Summary

The facility failed to maintain a clean, safe, and comfortable homelike environment for a resident with diagnoses including heart failure, type 2 diabetes mellitus, hypertension, and bipolar disorder. The resident, who had intact cognition, was observed to have a non-functioning air conditioner in her room, a large puddle of water on the bathroom floor due to a leaking sink, and a hole in the tile near the bathroom door.
